The best recent example of this is the LA wildfires. Despite the fact that it was 100% predictable and almost 18k homes and buildings have been destroyed or damaged, NO ONE has been held accountable.
The way you design an effective organization is to have exactly one person in charge of a specific aspect of a project: directly responsible individual (DRI) (Apple's term). When something goes wrong, that person is held accountable.
An accountability sink is the exact opposite of that. The government of California and LA is chock full of accountability sinks. Unfortunately, Davos is probably a lot more similar to California's government than to Apple.
